What does leave your phone
Scans leave only when you share them, and then they go wherever you send them. Your iPhone's own backups follow your iCloud settings. For subscriptions, Apple and our subscription provider receive the device and purchase details needed to manage Pro; your documents are never part of that. The privacy policy has the details.
